Applying for a place at Martha Trust
Before accepting a new resident into one of our homes we make every effort to ensure that they are compatible with existing residents.

CRITERIA FOR LIFECARE & RESPITE ADMISSIONS
To be considered for residence an individual must meet the following criteria:

  • They must be over 18 years of age.
  • They will have a learning disability ranging from moderate to profound
  • They will be unable to walk unaided.
  • In addition to learning disabilities, prospective residents will have moderate to profound physical disabilities and associated conditions
  • Prospective residents who exhibit behaviour that is likely to be disruptive, dangerous or distressing to existing residents will not be accepted.
PROCESS
We accept referrals from family members, care managers, community nurses, doctors or other relevant party.  Once a referral has been received the following process is initiated:
  • One of our Home Managers will arrange to visit the individual at home.
  • During the visit, or if necessary, at a second visit, a full Nursing Needs Assessment will be made.  This involves gathering as much information as possible about the prospective resident from the individual themselves, their family, carers, advocate or representative.
  • When the Nursing Needs Assessment is complete the suitability of the applicant will be reviewed by the Home Manager and the Director of Nursing and a decision taken as to whether a placement can be offered.
  • If a placement is offered a member of Martha Trust’s care team will be assigned to the prospective resident.
  • Before admission to Martha Trust the assigned care team member will visit the prospective resident at home.
  • The prospective resident will have an opportunity to get to know Martha Trust, existing residents and staff of their new home before admission. They will be invited to visit for varying lengths of time such as a meal, a morning or afternoon, an overnight stay and a weekend.
  • During these visits, information for the individual’s Life Plan (movement and handling assessment, consent form, skin integrity assessment etc.) will be collected in preparation for admission.
  • Once the prospective resident feels ready to move into their new home arrangements will be made for an admission date.
  • A Life Plan for the new resident will have been prepared and its recommendations will be put into effect on admission.
  • To ease the transition, arrangements can be made for a close family member, friend, carer or representative of the new resident to stay at Martha Trust in the early days of residence.
