Applying for a place at Martha Trust
Martha Trust exists to provide life care and respite care for people with profound and multiple disabilities. 
It is our intention that anyone moving into Martha Trust homes should be compatible with the existing client group and should be made to feel as welcome and comfortable as possible.

CRITERIA FOR LIFECARE & RESPITE ADMISSIONS
To be considered for residence at Martha Trust, an individual must meet the following admission criteria:

  • Prospective residents must be aged over 18 years of age.
  • Prospective residents will have a moderate to profound learning disability.
  • Prospective residents should be non-ambulant.
  • In addition to a learning disability, prospective residents will have moderate to severe physical disabilities and associated health care needs.
  • Prospective residents would be precluded from admission if they were to display behaviour likely to be particularly disruptive, dangerous or distressing to existing residents.
PROCESS
Martha Trust operates an ‘open’ referral system, i.e. initial referrals can be taken from family members, care managers, community nurses, medics or other relevant sources.  Once a referral for admission has been received, the following process is initiated:
 
  • The Home Manager will arrange to visit the individual in their home or current placement.
  • During the initial visit or at a second visit (if necessary) a full nursing needs assessment will be completed.  This involves gathering as much relevant information as possible about the prospective resident from the individual themselves, their family, carers, advocate or representative.
  • When the nursing needs assessment is complete, the suitability of the applicant will be discussed by the Home Manager and the Director of Nursing and a decision taken as to whether or not a placement can be offered.
  • If a placement is offered, a key worker and named nurse may be allocated to the prospective resident as appropriate.
  • The key worker and named nurse will arrange at least one visit to the person’s home in advance of admission to Martha Trust.
  • The prospective resident will always be offered the opportunity to familiarise themselves with Martha Trust and the existing residents and staff in advance of admission. This will involve a series of visits, which will range from short duration visits, visits for meals, all day visits, overnight stays and weekend stays.
  • During these visits, necessary Life Plan documentation (e.g. movement and handling assessment, consent form, skin integrity assessment etc) can be collated in preparation for admission.
  • Once the prospective resident feels comfortable to move into the home, arrangements will be made for an admission date.
  • On the day of admission, a basic Individual Life Plan will already have been prepared and the ‘life planning process’ will commence with immediate effect.
  • In order to ease the transition for the individual from one environment to another, facilities can be made available for a close family member or friend/carer/representative to stay over at Martha Trust in the early days of residence should this be thought beneficial to the new resident.
